With the 9.1.965 Adaptiva build and greater, the Health Check forms has a new format. Because of this, you may need to migrate your customized Health Check forms to the updated format in Adaptiva OneSite Health.
Use the following Solution details to migrate the Health Check forms to the redesigned format.
You can migrate your Health Check forms to the new format using the following steps:
Copy JSON data to save to your computer using the following steps:

Select Health Check Forms on the left navigation menu of OneSite Health.

Select a Health Check form:

Select a folder from the left-side column of the Health Check Forms dialog to choose a Health Check form.

Choose one of the methods below to search for a Health Check form:

Use the navigation tools on the bottom right to scroll through the pages to find and select a Health Check form.

Enter a search term on the search line, and select Search to find, and select a specific Health Check form.

Copy the JSON from a Health Check form:
-
Select Copy JSON in the upper-right corner of the Form dialog to copy the JSON data.
-
Copy and paste the JSON data into an application (such as notepad) on your computer.

Repeat step 5 for all the forms you want to copy the JSON data from.

Upgrade to the latest Adaptiva build. For more information, see Adaptiva OneSite Platform Installation User Guide.
Import your saved JSON data into a new Health Check form using the following steps:

Before you can import your saved JSON data, you must Upgrade Your Adaptiva OneSite Platform to the latest build.

Select Forms on the left navigation menu of OneSite Health to open the Web Forms dialog.
This opens the Web Forms dialog:

Create a new Check Health form:

Select + New on the upper-right corner of the Web Forms dialog to open the Forms dialog.
This opens the Forms dialog:

Enter a Name and a detailed Description in the General Settings pane.
Note
A red asterisk next to a field name indicates a required field.

Import your saved JSON data:

Select More > Import Form JSON to open the Import Form JSON dialog.
This opens the Import Form JSON pane:

Copy your saved JSON data from the file on your computer, and paste the data into the Form JSON pane.

Select OK in the lower-left corner of the Import Form JSON dialog.
This pre-populates the form data into the Form Data pane and Form Configuration workspace.

Select Save in the upper-left corner of the Forms dialog.
Set a new Health Check form using the following steps:

Select Health Checks on the left navigation menu of OneSite Health.
This opens the Health Check dialog:

Select on the Health Check that you want to set the new Health Check form to.
This opens the Form dialog:

Scroll down to the Health Check Parameters section, and select +Set Form.
This opens the Add Web Form dialog:

Select the the new Health Check form checkbox, and select Set Form.
This pre-populates the form data into the Health Check dialog.

Select Save in the upper-left corner of the Health Check dialog.
Repeat steps 2 through 5 to set the new Health Check form to each desired Health Check.
Configure a new Health Check form in Health Policies using the following steps:

Select Health Policies on the left navigation menu of OneSite Health to open the Instant Health Policies dialog.
This opens the Instant Health Policies dialog:

Select a Health Policy in the Instant Health Policies table to open the Policy dialog.
This opens the Policy dialog:

Scroll down to the Health Checks section, and select the ellipses (...).

Select Open Health Check Form from the menu to open the dialog.
This opens the Health Check Form dialog.
- Make any necessary changes, and select OK to update the form.
